The ProFusion Form Builder is an easy way for you to design and build custom forms for your website. Create contact us forms, web estimate forms, comment cards, application forms, order forms and more.

Features:

• Autoresponder - Automatically send custom email to your users after they complete the form on your website.
• Thank you page - After completing the form, your users can be directed to a "thank you" page with your custom message.
• Field types:
  • Text - User enters text into a field, such as first name or last name.
  • Multi-line text box - User enters longer text, such as questions or comments. Drop-down list - User selects one of several options from a list that appears only when the control is selected.
  • Checkbox list - User selects one or many of several list options that are always visible.
  • Radio button list - User selects only one option from list options that are always visible.
  • Form description - Place text above and below your form to describe what the form is used for.
  • Database storage - After the user completes the form, you have the option of automatically storing their information in the application database. This is a great way to build a mailing list!
  • Export form completed to an Excel spreadsheet for further data manipulation.
• Email notification - Specify whether email notification is automatically sent to the form owner when the form is completed.
• Custom look - You control the background color, font size, font style and font color for each individual form.
